There is really no affection or intimacy involved. Though 'love' is part of the ceremony and spouses will usually address you as 'my love' or 'dear'.
In Skyrim - unlike most other RPGs, marriage is strictly a practical alternative. I think some people are taking this marriage idea out of proportion. And the best thing? We barely need interact with the other types if we can ALL allow one another the personal space we would wish for ourselves. :-) Seriously people! Isn't it infinitely better to live and let live? If homosexuality is a sin in your eyes - let those other people sin! If intolerance is a sin in your eyes - let those other people sin! There's enough room for us ALL in this physical universe!. 2) People *should* allow others their own space - and neither homosexuality OR heterosexuality would be metaphorically shoved down anybody elses' throats. 1) People *should* marry for love - regardless of gender, race, or creed/religion. And like a few people already said, if it's something that makes you uncomfortable all you have to do is NOT DO IT - and then you'll never have to think about it outside of Skyrim! -P Honestly, there are many more opportunities for different types of roleplaying since they enabled marriage of ANY gender.
